Penile Fillers for Premature Ejaculation: How Can They Improve Control and Confidence?

Premature ejaculation can make a man feel that he has little control over his sexual performance. Over time, this can affect confidence, intimacy and the overall sexual experience.

How does penile filler treatment help premature ejaculation?  

The glans contains many sensory nerve endings and plays an important role in sexual sensation and ejaculation.

In some men, high sensitivity may contribute to rapid ejaculation. Hyaluronic acid injection adds a small amount of volume beneath the surface of the glans. This can reduce the intensity of tactile stimulation reaching the sensory receptors.

The treatment does not remove sensation. Instead, the goal is to reduce excessive sensitivity while maintaining normal sexual sensation.

This may give a man more time between the beginning of sexual stimulation and ejaculation.

Research supports this mechanism. The European Association of Urology reports that randomized studies have found a modest but significant increase in ejaculation time following HA glans injections.

How is the treatment performed?  

I begin with a detailed consultation.

I assess the patient’s ejaculation history, sexual control, erection quality, previous treatments and expectations. I also look for conditions that may contribute to premature ejaculation.

Once I determine that HA injection may be appropriate, the treatment is performed using a local anaesthetic to keep the procedure comfortable.

I then inject a measured amount of hyaluronic acid into carefully selected areas of the glans. The injection technique and amount depend on the patient’s anatomy and treatment plan.

The aim is not to add excessive volume. I focus on controlled treatment and an even result.

After treatment, I provide instructions regarding recovery and when sexual activity can be resumed.

What results can I expect?  

The results can vary between patients, but clinical research has shown encouraging improvements.

In a randomized controlled study involving 30 men, HA injection into the glans produced a significant improvement in intravaginal ejaculation latency time compared with saline injection.

Another study followed 80 men with persistent premature ejaculation for six months. The HA group showed significant improvement in ejaculation time, while satisfaction with sexual intercourse reached 78.4% at six months.

A separate pilot study found that average ejaculation time increased from approximately 38 seconds before treatment to 323 seconds one month after treatment. Although the effect reduced over subsequent months, it remained above the original baseline at 12 months.

These results are encouraging, but I always explain that clinical results differ from one man to another.

Can better ejaculation control improve confidence?  

Yes, and this is an important part of treatment.

When a man repeatedly ejaculates sooner than he wants, he may start worrying about it before intimacy even begins. That worry can affect his confidence and make sexual experiences more stressful.

If treatment helps him gain better control, he may feel more relaxed and confident with his partner.

Better control can mean more than extra time. It can mean feeling confident again.

Is penile filler a permanent treatment?  

Hyaluronic acid is a temporary filler. The body gradually breaks it down, so the effect can reduce over time.

This means I can discuss the expected duration and future treatment options with the patient rather than treating the procedure as an irreversible change.

Is this treatment suitable for every man?  

No. I believe proper assessment is essential.

I first identify the type of premature ejaculation and check for other concerns, including erectile dysfunction, urinary or genital problems and psychological factors.

The EAU currently recommends established treatments such as dapoxetine or lidocaine/prilocaine spray as first-line options for lifelong premature ejaculation. It considers HA injection an option that should be used with caution because more safety and long-term evidence are needed.

For some men, HA injection may be considered when other approaches have not provided satisfactory results or when a minimally invasive option is being discussed.

What should I expect after treatment?  

Temporary swelling, tenderness or mild discomfort can occur after injection. I provide specific aftercare instructions and monitor the patient’s progress.

It is also important to understand that the goal is better control, not complete loss of sensitivity.

I want my patients to continue experiencing normal sexual pleasure while gaining better control over ejaculation.
